Clinical Transformation Strategy & Planning Director

The Clinical Transformation Strategy & Planning Director will develop and execute enterprise transformation strategies and initiatives to deliver an exceptional consumer experience and drives a standardized and action-oriented clinical operations model.

Primary duties include, but are not limited to:

  • Deliver value for customers, communities and stakeholders through clinical innovation and efficiency initiatives.

  • Developing and improving various business processes through cooperative engagement with business leaders accountable for transformational change, strategy development and change management leadership to include product ownership for the care delivery solutions consisting of developing and implementing roadmaps and strategies to ensure that all stakeholders are aligned, and clinical model design ensures quality outcomes for patients

  • Partners with senior and executive leadership to sponsor innovative solutions that create efficient ways to simplify end to end processes to achieve clinical excellence objectives.

  • Synthesizes information into meaningful executive summaries with actionable next steps to ensure leadership has correct and timely information. Comfortable with executive level presentations.

  • Conducts research by engaging enterprise partners to ensure championship of options and solutions.

  • Serves as a lead on the team - interviewing and recommending candidates, prioritizing, and balancing the team's workload, and supporting day-to-day activities.

  • Collaborates with subject matter experts across the organization to build innovative care delivery solutions and position the organization for success in value and risk-based payment models

  • Support digital health initiatives, including strategic initiatives and seamless integration into clinical models and applications

  • Some travel may be required.

Qualifications

Minimum Requirements:

  • Requires a BSN/MSN degree in Nursing, BA/BS degree in Information Technology, Computer Science or related field of study with 10+ years` experience in Strategic Planning, Project Management and, or Business Process Design, chronic care and palliative care operations; or any combination of education and experience, which would provide an equivalent background.

  • Requires clinical experience in caring for chronic complex populations, palliative and/or hospice care.

    Preferred Qualifications:

  • Preferred: MBA or MPH or other advanced degree
